At the Cardinal Cushing Centers, we support people with intellectual and developmental disabilities, providing possibilities and opportunities for growth and advancement. Our programs for students, adults and community members include education, employment training, transition services, residential care, therapeutic and recreational services.
Provide nursing supports to the Adult Family Care program as outlines in the responsibilities listed below.
PRIMARY DUTIES AND RESPONSIBILITIES COMMON TO THIS JOB DESCRIPTION

General

Regular attendance and promptly reporting to work at the beginning of the scheduled shift are essential functions of this position

The responsibilities of the registered nurse include:
1. Completing or coordinating all applicable clinical assessments and clinical evaluations, provided that only a registered nurse can complete such assessments and evaluations;
2. Developing the member’s interim and final AFC plan of care, with input from the member or responsible party, all members of the MDT, and other individuals designated by the member;
3. Completing a semi-annual health status report for each member and fax to PCP;
4. Ensuring implementation of the AFC plan of care;
5. Coordinating the delivery of AFC with any other health services or supportive services the member is receiving from MassHealth or other agencies or organizations including, but not limited to, visiting nurse services, therapy services, Department of Developmental Services (DDS) services, Department of Mental Health (DMH) services, and Massachusetts Rehabilitation Commission (MRC) services;
6. Conducting on-site visits with each member at the qualified setting:
a. for members authorized for AFC level I service payment, the nurse must conduct on-site visits every other month, or more often as the member’s condition warrants
b. for AFC level II service payment, the nurse must conduct on-site visits every month, or more often as the member’s condition warrants
7. Completing a nursing progress note for each on-site visit or encounter and upon significant change;
8. Monitoring each member’s health status and documenting those findings in the member’s medical record for each on-site visit or encounter, or more often as the member’s condition warrants;
9. Educating the member about hygiene and health concerns;
10. Reporting changes in the member’s condition to the member’s PCP;
11. Coordinating and implementing the PCP form and approval for AFC with the member, AFC caregiver, and AFC provider personnel;
12. Developing, in conjunction with the MDT. the AFC caregiver and the member or responsible party, an emergency backup and personal care contingency plan for each member that includes an alternative plan for the member if the AFC caregiver is temporarily unable to provide care; and
13. Overseeing, monitoring, supporting, training, and evaluating AFC caregivers.
The responsibilities of the care manager include:
1. conducting the initial and ongoing assessments of the AFC qualified setting;
2. may be asked to take on the role of a Community Health Worker, as needed;
3. obtaining a social history and conducting of a psychosocial assessment and evaluation;
4. assist with the selecting, training, evaluating, and supervising of the AFC caregivers in conjunction with the registered nurse;
5. participating in the development, implementation, and ongoing review of the AFC plan of care;
6. assist with the implementation of any CARF and MassHealth regulations
7. conducting on-site and or telehealth visits with each member at the qualified setting bimonthly (alternating with the bimonthly visit by the registered nurse) for Level I, and monthly for Level II according to current AFC regulations;
8. completing a care-management progress note corresponding with each on-site visit or encounter and upon significant change;
9. assisting with obtaining information and accessing other health-care, mental health, community and DDS services.
10. reviewing and documenting the fire drills and safety procedures for the qualified setting;
11. participating in discharge planning and implementation
12. reviewing at least annually the suitability of the qualified setting
Providing ongoing training to AFC caregivers on health and aging
Willingness to use your personal automobile to get to and from home and program sites.

Documentation
Maintains accurate time records; collecting, organizing, evaluating information and submitting information as required
Communicates effectively in writing and records information neatly, coherently, accurately and objectively
Organizes record keeping systems and manages documentation efficiently
Documents significant activities, events and incidents and follows through on communication to others
Supervises staff in the implementation of organizational policies and procedures
Related Work Experience
The registered nurse must be fully licensed by the Massachusetts Board of Registration in Nursing. The registered nurse must have at least two years’ recent experience in the direct care of the elderly or disabled adults.
Formal Education, Certification, Licensing
A. Current MA license
B. Graduate of NLN approved school of nursing
C. A valid MA driver’s license
D. Completion of orientation and required training
E. Fluent in both English and a second language preferred
